Assumption of Risk

I acknowledge that participation in golf activities involves inherent risks, including but not limited to:

• Being struck by a golf ball or club

• Uneven terrain, slips, trips and falls

• Weather-related risks (heat, lightning, etc)

• Travel to and from tournament venues

I voluntarily accept all risks associated with participation.​

Release and Waiver of Liability

In consideration for participation, I (on behalf of myself and/or my child) hereby release, waive and discharge Moon Golf Junior Tour, LLC, it's directors, officers, staff, volunteers, host facilities, sponsors and affiliates from any and all liability for injury, illness, loss or damage arising from participation, except where prohibited by law.

Medical Treatment Authorization

In the event of injury or medical emergency, I authorize Moon Golf Junior Tour to obtain medical treatment deemed necessary. I understand that I am financially responsible for any medical expenses incurred.

Code of Conduct

Participants are expected to demonstrate respectful behavior, good sportsmanship, and compliance with rules and instructions. Moon Golf Junior Tour reserves the right to remove any participant or parents for misconduct without refund.

Weather Policy

Tournaments may be delayed, suspended, or canceled due to weather conditions. The tours decision regarding delays or cancellations is final.

Equipment Responsibility 

Participants are responsible for their personal equipment. Moon Golf Junior Tour is not liable for lost, stolen, or damaged items.

Refund & Cancellation Policy

Entry fees are non-refundable except as determined by Moon Golf Junior Tour Policy (example: event cancellation, medical withdrawal, etc.).

Media & Photography Release

I grant permission for photographs/videos of the participant to be used for promotional, marketing, and educational purposes without compensation.
